Online Court Assistance Program - Document Preparation.

The Utah Online Court Assistance Program (OCAP) is the official State of Utah website for assistance in preparing court documents if you are not able to have an attorney draft them for you. Documents can be prepared for divorce, landlord tenant, guardianship of a minor, or protective orders. You may use this program for free. If you file documents created by the program with the court, a $20.00 fee in addition to any court filing fees will be charged at that time. This program is a service of the Utah Courts and State Legislature.

These programs are designed to be easy to use. You simply point and click or type in your information. There are brief explanations about the law that relates to your documents and instructions on how to use the documents you prepare. At the end of each program, you will be able to print out documents that look much like those an attorney would prepare.

Not all documents required for all types of court cases can be prepared using these programs. Please read the description of each document preparation program carefully to be sure it addresses your needs. Information on obtaining legal assistance can be found here.

Divorce (Document preparation for marriage dissolution, custody, parent time, support and asset distribution options)

Divorce Stipulation (Document preparation for previously filed divorces only. This program should only be used following successful mediation or resolution of issues in the original divorce petition.)

Parentage (Document preparation for child-support, custody, parent-time, paternity, but not divorce)

Protective Orders (Document preparation for court-ordered protection)

Civil Stalking (Document preparation for civil stalking forms)

Guardianship of a Minor (Document preparation for establishing guardianship of a child under 18. Do not use if you are a parent seeking custody, visitation or support.)

Guardian/Conservator Inventory and Annual Reporting (Document preparation for guardians and conservators to create the documents required by law to file periodically with the court following their appointment. Documents to terminate the guardianship and/or conservatorship if the ward is deceased may also be prepared.)

Landlord-tenant Cases (Document preparation for filing or answering an eviction complaint)

Employer's Answers to Garnishment Interrogatories (Document preparation for the worksheet and form for Answers to the Interrogatories for Writs of Continuing Garnishment)
